The Plaza Cinema 4 held its Grand Opening on October 18, 1974, after a pre-opening show at 12:01 AM on the 16th. United Artists assumed operation on August 1, 1986, after it acquired the Gulf States circuit. The theater was later operated by Movies Incorporated and then the Silver Cinemas division of Landmark Theatres. After a two-year lease by an independent operator, the theater closed shortly before the Grand 12 opened adjacent to the shopping center in 2002.
After Hurricane Katrina flooded the area, the Lake Forest Mall was demolished.
